Among the contributors credited with shaping this guideline is Dr. Gabrielle Lyon, a physician widely known for her work in metabolic health and longevity medicine. Her involvement places LifeSpanMD’s orbit of expertise directly inside a policy document that will affect how hundreds of thousands of service members are screened and treated for hormone-related symptoms.

The core problem the guideline solves

Military populations face a unique diagnostic challenge. Sleep restriction, caloric deficit, intense training, and sustained operations all suppress the hormonal axis that regulates testosterone production, and this suppression usually resolves within days once a service member recovers. That temporary dip looks like hypogonadism on a blood test, but treating it as a permanent deficiency would expose otherwise healthy individuals to unnecessary therapy and risk their fertility. The guideline addresses this directly by requiring specific pre-test conditions before any specimen is considered valid, including a thirty-day buffer after extreme training or illness.

What the new standard requires

Screening at the Periodic Health Assessment for all men age thirty and older

Two morning, fasting testosterone draws on separate days below three hundred nanograms per deciliter, paired with a compatible symptom profile

A mandatory baseline hematocrit before any treatment begins

Documentation of fertility intent before writing any prescription, since exogenous testosterone can suppress sperm production

A defined trough target of three hundred to six hundred nanograms per deciliter, with reassessment of the diagnosis if six months of therapy at target produces no symptom benefit

Why Dr. Lyon’s contribution matters

Dr. Lyon has built her reputation on a disciplined, evidence-first approach to metabolic and hormonal health, and that sensibility is visible throughout the guideline. The document is notably conservative about what testosterone therapy can and cannot do, citing the Testosterone Trials showing no benefit for vitality or walking distance, and stating plainly that guidance built around performance enhancement rather than a diagnosed deficiency is not supported by current evidence. That kind of rigor, insisting on both a lab-confirmed deficiency and a compatible symptom profile before treating, reflects the same standard LifeSpanMD applies across its own network of practices.

Why this matters beyond the military

For a field crowded with anecdote and confident marketing claims, this guideline is a useful benchmark for how testosterone deficiency should be evaluated anywhere, not just in uniform. It also addresses erythrocytosis management, deployment considerations for service members on therapy, and fertility-preserving alternatives like hCG and clomiphene for those who are not ready to close the door on having children. Two people can have the same BMI while having dramatically different levels of cardiovascular risk and physical fitness.

That’s why body composition has become a far more valuable measurement in preventive and longevity medicine.

Rather than focusing only on total weight, body composition evaluates what the body is actually made of. It measures lean muscle, body fat, visceral fat, bone mass, and other factors that provide a much clearer picture of overall health.

This distinction matters because not all weight affects the body equally.

Muscle supports metabolism, improves insulin sensitivity, protects mobility, and plays a critical role in healthy aging. While it has long been associated with athletic performance, research now consistently identifies it as one of the strongest predictors of longevity, cardiovascular health, and overall resilience. It reflects how well your heart, lungs, blood vessels, and muscles work together to support the body under physical demand.

That makes it far more than a fitness metric. It offers a meaningful window into how well the body is aging.

Higher levels of cardiorespiratory fitness have been linked to lower rates of cardiovascular disease, metabolic dysfunction, cognitive decline, and premature mortality. Just as importantly, improving VO₂ max can reduce health risks even when body weight remains unchanged. This reinforces an important principle in longevity medicine: fitness often matters more than the number on the scale.

VO₂ max also influences everyday life more than many people realize. Walking up a flight of stairs, recovering after physical activity, keeping pace with children or grandchildren, or maintaining energy throughout a demanding workday all depend on the body’s ability to deliver and use oxygen efficiently. As that capacity declines, everyday activities become more difficult long before disease becomes apparent.
Unlike many markers of aging, VO₂ max is highly adaptable. Regular cardiovascular exercise, interval training, and strength training all improve the body’s ability to transport and utilize oxygen. Even modest gains can translate into meaningful improvements in long-term health.

While many wearable devices estimate VO₂ max, formal exercise testing provides a more accurate assessment and establishes a baseline that can be tracked over time. More importantly, it helps physicians identify opportunities to improve cardiovascular fitness before symptoms develop.

Longevity isn’t simply about adding years to life. It’s about preserving the capacity to live those years well. VO₂ max represents one of the clearest measurements of that capacity, making it one of the most valuable metrics for anyone focused on long-term health.

["post-14046","articles","type-articles","status-publish","has-post-thumbnail","hentry

VO₂ Max: One of the Strongest Predictors of Longevity - SoapMD

<meta property="og:description" content="Most people judge fitness by what they can see. It progresses gradually over many years, often without causing symptoms until significant damage has already occurred.

That makes early detection one of the most valuable tools in preventive medicine.

A coronary artery calcium (CAC) score helps physicians identify evidence of plaque buildup before cardiovascular disease becomes apparent. Rather than estimating future risk based solely on traditional risk factors, calcium scoring provides a direct look at whether calcified plaque has already formed within the coronary arteries.

The scan itself is quick, non-invasive, and requires no injections or recovery time. In just a few minutes, it can provide valuable insight into a person’s cardiovascular health long before symptoms develop. It does involve a limited amount of radiation, but much less than a CT scan.
This is particularly important because cholesterol levels alone do not tell the entire story.

Some people with elevated cholesterol never develop significant coronary artery disease, while others experience heart attacks despite cholesterol levels that appear normal. Genetics, inflammation, blood pressure, metabolic health, and lifestyle all influence cardiovascular risk. Calcium scoring adds another layer of information by identifying whether plaque is already present.

For many patients, the results help guide more personalized decisions. A score of zero may reinforce that current prevention strategies are working well. A higher score may prompt additional testing or earlier intervention through lifestyle changes, cholesterol management, or additional cardiovascular evaluation.

CT Calcium scores only find hard calcified plaque. In the rare case where a patient has just soft plaque, it can be falsely reassuring. This is a small number of patients. The majority of patients with coronary diseases have a combination of soft and hard plaque.

The value isn’t simply knowing a number. It’s understanding what that number means within the context of your overall health.
Not everyone needs a calcium score, and it should never replace a comprehensive cardiovascular assessment. Age, family history, existing medical conditions, and individual risk factors all influence whether the test is appropriate. Like any diagnostic tool, it delivers the greatest value when interpreted alongside the broader clinical picture.

Preventive medicine continues to shift away from reacting to disease and toward identifying it earlier, when intervention can have the greatest impact.

Calcium scoring reflects that philosophy. Instead of waiting for symptoms to appear, it offers an opportunity to better understand cardiovascular health while there is still time to change its trajectory.

["post-14043","articles","type-articles","status-publish","has-post-thumbnail","hentry

Calcium Scoring Explained: Detecting Cardiovascular Risk Before Symptoms Begin - SoapMD

<meta property="og:description" content="Heart disease rarely develops overnight. Patients celebrate when the number on the scale goes down and worry when it goes up. While body weight can provide useful information, it reveals very little about what truly influences long-term health.

What matters far more is what makes up that weight.

Muscle is one of the body’s most important organs for metabolic health. It helps regulate blood sugar, supports insulin sensitivity, protects bone health, and plays a central role in movement, balance, and recovery. As we age, maintaining muscle becomes one of the strongest predictors of independence, resilience, and overall healthspan.

This is why weight loss alone should never be the goal.

Many people lose both fat and lean muscle during periods of rapid weight loss, particularly when strength training and proper nutrition are overlooked. While the scale may reflect success, losing muscle can slow metabolism, reduce physical function, and make it more difficult to maintain long-term results.

The objective isn’t simply to lose weight. It’s to improve body composition while preserving the muscle that supports lifelong health.

Muscle also serves as one of the body’s greatest defenses against age-related decline. Beginning in midlife, adults naturally lose muscle mass unless they actively work to maintain it. Over time, this contributes to reduced strength, slower recovery, impaired mobility, and a greater risk of falls and chronic disease.

Fortunately, these changes respond remarkably well to consistent lifestyle choices. Resistance training 2-3 times a week, adequate protein intake, quality sleep, and regular physical activity all help preserve lean muscle while supporting healthier metabolism.

This is one reason body composition has become a far more meaningful measurement than body weight alone. Two people may weigh exactly the same yet have dramatically different levels of muscle, body fat, and metabolic health. Looking beyond the scale provides a much clearer understanding of overall physiology and long-term risk.

Weight will always be part of the conversation, but it should never be the entire conversation.

The goal isn’t simply to become lighter. It’s to build a stronger, more resilient body that supports energy, movement, and vitality for decades to come.

["post-14040","articles","type-articles","status-publish","has-post-thumbnail","hentry

Why Muscle Matters More Than Weight - SoapMD

<meta property="og:description" content="For decades, weight has been one of the most common ways to measure health.
