Patients may contact the LifeSpanMD Breast Center directly or be referred by their physician.
Complete our online appointment request form or call our team. Physicians can also submit a referral and relevant medical records directly to the Breast Center.

Speak With Our Patient Care Team
A member of our team will contact you for a brief consultation call to understand what you need, answer questions, review any prior imaging or referral information, and determine the appropriate next step.
If records or previous breast imaging are needed before your visit, we’ll help coordinate them.

Schedule Your Visit
Once we understand your needs, we’ll schedule the appropriate screening, imaging or diagnostic appointment.
Our goal is to make the entire experience straightforward, from your first conversation through imaging, results and any necessary follow-up care.
For Referring Physicians: A More Personal Approach for Your Patients
The LifeSpanMD Breast Center welcomes physician referrals for screening, diagnostic imaging and further evaluation.
Our team works closely with referring providers to make the process easy for both the physician and patient. Once a referral is received, our patient care team will contact the patient directly, coordinate records and prior imaging when needed, and schedule the appropriate visit.
Dr. Hadar provides patients with timely, direct communication regarding their results, while helping ensure referring physicians remain informed throughout the process.
